 PROVISIONS on Tender for “Creation of National Anti-tuberculosis Health Improvement Center for Psychological and Social Rehabilitation of Children Treated against TB”

SECTION 1. General Provisions
 
1.1. The Foundation for Development of Ukraine (hereinafter referred to as “Customer”) announces an open competition aimed to select the best proposals from health improvement and treatment institutions of Ukraine and the Republic of Crimea for creation of the national Anti-tuberculosis center for psychological and social rehabilitation of children that have undergone TB treatment.
1.2. Place and deadline for applications submission: applications are submitted by bidder (hereinafter referred to as Participants) to the following address: Foundation for Development of Ukraine, 29g Yaroslavov Val Street, Kiev, 01034, Ukraine; before 18:00 (Kiev time) on 31 October, 2007.
1.3. Procedure of applications submission: in person, by post or express mail. If the bid has been sent by post or express mail, it must reach the Foundation’s address by the date specified in the Cl. 2.
1.4. Time of applications opening: 10:00 (Kiev time), 1 November, 2007.

1.6. The Participant covers all expenses related to preparation and submission of his or her application, and the Customer is in no case responsible for these expenses irrespective of the competition results.
1.7. Applications shall be prepared in Ukrainian or Russian.
1.8. The winner will be announced before 1 November 2007. The winner will be announced on the Customer’s website and in the media.
1.9. The Customer will conclude agreements with the competition winners which will regulate terms and conditions of projects execution and funding.
 
SECTION 2. Tender organizer

2.1. The tender is organized by the Foundation for Development of Ukraine in partnership with Ukrainian Ministry of Health Care.
2.2. The tender organizer ensures:
• equal conditions for all Participants;
• transparency of the winner selection procedure and unbiased attitude of the tender committee towards the Participants;
• protection from disclosure of information about tender results before the date of the official announcement.
 
SECTION 3. Winners selection criteria

 
3.1. The tender shall comprise one round without preliminary selection of Participants and qualification requirements.
3.2. Selection criteria:
• Following a review of application forms of Participants (see Appendix 1).
3.3.  Applications will be considered by the tender committee that will provide an expert analysis from the specialists in the area of TB, health improvement and resort treatment, and rehabilitation.

SECTION 4. Tender committee
 
4.1. The tender organizer shall establish the committee and approve its members.
4.2. The committee shall consist of:
• Customer’s representatives;
• Representatives of the Ukrainian Ministry of Health Care;
• A representative of Ukraine 3000 Charitable Foundation;
• Representatives of other non-governmental organizations.
4.3. Detailed information about the members of the tender committee will be additionally placed on the Customer’s website in October, 2007.
4.4. Voting by the tender committee members shall take place in one round.
4.5. Review of applications by the committee members starts once submission of applications has been over (see Cl 1.4. of Section 1).
4.6. Assessment of applications is carried out by every member of the tender committee individually in accordance with application form (see Appendix 1). 
4.7. Voting is carried out by every member of the competition committee (scale of 1-10 points) with regard to every section of the application form (see Appendix 1).
4.8. The final assessment of every Participant covers the total assessments of all the members of the tender committee on every section of the application form (see Appendix 1).
4.9. The committee shall determine and select one winner who will get support and funding within the Foundation’s budget allocated by the Foundation on creation of national Anti-tuberculosis  health improvement center for psychological and social rehabilitation of children treated against TB.        
 
 
SECTION 5. Guidelines for Bidders
 
5.1. Procedure of applications submission:
5.1.1.      Applications are submitted by Participants within the timeline specified in Cl. 1.2. of Section 1.
5.1.2.      The Customer reserves the right to make decisions about extension of the application submission timeline before termination of the previous period set for submission of applications (information about the timeline changes will be placed on the Customer’s website). In that case, period of validity of all competition terms and conditions is extended including a new applications submission deadline.
5.1.3.      A Participant can submit only one application.
5.2. Preparation and submission of applications:
5.2.1.      A tender application must cover the sections specified in the application form (Appendix 1).
5.2.2. Application format: up to 10 pages, font 12 (Times New Roman preferably) in a text editor (Microsoft Word preferably).
5.2.3. Application (with all documents) shall be enveloped, sealed and marked as follows: Tender Name, to Tatyana Bilyk, Foundation for Development of Ukraine, Foundation’s Address (according to the Cl. 1.2. of Section 1).
5.3. The Customer shall confirm receipt of the application from the participant and the date thereof, should the Participant make a written inquiry.
5.4. Only questions received in a written form, via e-mail or fax are reviewed. The questions received in a written form and answers will be placed on the Customer’s website to ensure transparency of the tender.
5.5. Applications and all related documents shall be written in Ukrainian or in Russian.
5.6. Participants shall bear the risk of failure of his/her application to meet the deadline or non-reception of the application by the Customer.
 
SECTION 6. Tender Results
 
6.1. Tender results shall be summed up from 1 November to 30 November, 2007.
6.2. The winner will be announced before 1 December, 2007.
6.3. The winner will be announced on the Customer’s website and in the media.

Note: The concept is prepared in a free format. It must feature a realizable development plan for the Center in terms of adoption of new and effective forms and methods of children’s rehabilitation. The Foundation plans to finance development and adaptation of new rehabilitation programs. However, the winner is expected to provide ideas and plans regarding probable innovative rehabilitation programs, projects on healthy style of living, and other aspects  important for children and youth that will help them rehabilitate and be healthy in the future.
