How to get humanitarian aid?

ADULT FOOD PACKAGES

1. Who can receive this aid?

Government-controlled areas

Survival kits are issued only to IDPs registered according to the established procedure and belonging to the following categories:

  • people with the 1st and the 2nd disability groups
  • single parents with children under 18 (including widows/widowers)
  • families with many children (3 or more children, all of them are under 18)
  • retired people older than 65
  • families raising disabled children
  • families with adopted children, orphans and children, whose parents have their rights annulled
  • disabled veterans of the Great Patriotic War (World War II); people, who obtained disability due to liquidation of accident consequences at Chernobyl nuclear power plant and those equated to disabled war veterans; disabled veterans of Afghanistan war
  • people, who are registered in district administrations for labour and social protection of the population as low-income individuals
  • pregnant women of any gestational age that are registered according to the established procedure
  • orphaned children under 23 years old

Contact line

Packages are issued only to the following categories of local residents and IDPs, who are registered according to the established procedure:

  • people with the 1st and the 2nd disability groups

  • babies from 0 to 35 months old (children survival kits)

The operation of Rinat Akhmetov Humanitarian Center in non-government-controlled areas was blocked on February 28, 2017, and later it was forbidden. The Center is ready to renew the issuance of humanitarian aid in non-government-controlled areas, as soon as it becomes possible.

3. How to apply for humanitarian aid?

To register for survival kits receipt it is necessary to come to the issuing point, corresponding to the place of residence or stay (for IDPs) with the full package of documents (originals only, copies are not accepted).

4. What documents shall be provided?

HUMANITARIAN AID CAN BE RECEIVED ONLY UPON AVAILABILITY OF ALL ORIGINAL DOCUMENTS

Birth certificate of a child with no information about the father (blank) or with a reference to Article 135 of the Family Code of Ukraine.

An alternative document is: "An extract from the State Civil Register of births according to the first part of Article 135 of the Family Code of Ukraine for aid designation".

Widows/widowers:

Passport.

Child's birth certificate.

Death certificate of one parent.

Multi-child families

(with 3 children, all of them are under 18)

Parents' passports.

Operating multi-child family certificate.

Retired people of 65 and older

Displaced families with 1 or 2 children under 18, who are in difficult life circumstances. For Ukrainian or front-line territories only.

Passport of one of the parents.

Birth certificate or certificate of discharge from maternity hospitals.

Registration certificate of internally displaced persons, who temporarily leave ATO area.

Report on social inspection of family housing conditions (it should be mentioned that the family is residing in difficult income and living conditions) - issued by social service of town or settlement or another operating authority.

And in the settlements on government-controlled territory of Donbas survival kits are issued only upon availability of IDP certificate.

Humanitarian aid is issued to IDPs personally. Relatives, friends and acquaintances are not entitled to receive humanitarian aid for someone without additional document – power of attorney or relevant certificate from hospital about beneficiary's inability to receive humanitarian aid at the issuing points (certificate must be dated no earlier than a week before the date of survival kit receipt).

Kit content depends on the supply of products and it can be modified within the variants listed in the table.

6. How often are the survival kits issued?

Food packages are issued every 42 days at the Humanitarian Center's fixed issuing points. In the villages and towns without fixed issuing points the packages are issued according to the schedule of mobile teams’ visits (see paragraph 8 of this section).

The frequency of issuance can be changed towards the prolongation of term between supplies depending on supply volumes to the town/settlement.

IMPORTANT! The Humanitarian Center issues food products that help people to survive. The aid is issued by categories, directly to beneficiaries or their representatives within the intervals established by the Humanitarian Center. If a beneficiary does not come to pick up humanitarian aid during the day announced by the Humanitarian Center, his/her package is not stored, it is issued to other people who need it. Humanitarian aid is not accumulated and the number of packages during the next aid receipt is not increasing depending on the number of missed aid receipt cycles.

  • orphans and children deprived of parental care, residing in a family-type orphanage, with foster family, under custody; and abandoned children in hospitals

3. How to file an application for humanitarian aid?

Свидетельство о смерти одного из родителей

Дети из многодетной семьи (от 3 и более детей, возраст всех детей до 18 лет).

Действующее удостоверение многодетной семьи

Свидетельства о рождении всех детей

Семьи, которые воспитывают детей-инвалидов

Удостоверение ребенка-инвалида, которое выдает УТСЗН.

TARGETED MEDICAL ASSISTANCE

1. Who can receive this aid?

Targeted medical assistance can be provided to:

  • residents of Donetsk and Luhansk regions
  • internally displaced persons who have a corresponding certificate

Targeted medical assistance is provided to children, who belong to the following benefit-entitled categories:

  • families that bring up orphans and children under custody
  • multi-child families with 3 and more children under 18
  • single mothers or widows with children under 18 years old
  • families in difficult life circumstances (families, who have more than one severely ill family member, for example, mother with cancer and her child with ICP, or two disabled persons of the 1st group etc.)

Also, targeted medical assistance is provided to adult civilians, who belong to the following benefit-entitled categories:

  • a single mother/father or grandmother/grandfather-guardian
  • child and mother/father affected by hostilities, road accidents, fire, etc .
  • a young person between the ages of 18 and 25, who needs help for curable disease treatment

2. What diseases does the Humanitarian Center help to treat?

Children

Urgent life-saving surgeries (only upon availability of medical appointment for the operation with the list of required medications in hard copy):

  • injuries due to ATO
  • urgent life-saving surgeries
  • prosthesis

Urgent life-saving treatment (only upon availability of medical appointment for the operation with the list of required medications in hard copy):

  • severely ill
  • peaceful residents, injured due to ATO
  • rehabilitation equipment for injuries due to ATO (crutches, walking frame, canes, etc.)

Adults

Urgent life-saving surgeries (only upon availability of medical appointment for the operation with the list of required medications in hard copy):

  • urgent life-saving surgeries
  • prosthesis

Urgent life-saving treatment (only upon availability of medical appointment for the operation with the list of required medications in hard copy):

  • severely ill
  • rehabilitation equipment for injuries due to ATO (crutches, walking frame, canes, etc.)

3. How to request the assistance?

The requests for targeted medical assistance can be left via the Humanitarian Center's hotline 0800 509 001 or send an email to helpdonbass@fdu.org.ua.

Please note: assistance is provided only upon availability of medical certificate.

4. What documents shall be provided?

To be provided with assistance it is necessary to provide:

  • medical direction for surgery with the list of medications
  • document proving the benefit-entitled category (for adult civilians)

MEDICATIONS FOR CHILDREN

1. Who can receive this aid?

All children from 0 to 16 years old living in non-government-controlled areas of Donbas and internally displaced children who left for the peaceful territories of Donbass.

2. What diseases are the medications provided for?

  • diabetes mellitus
  • bronchial asthma
  • convulsive disorder and epileptic syndrome with the following diagnoses:
    • epilepsy (of any form)
    • cerebral palsy
    • hydrocephalus
    • traumatic brain injuries and their consequences
    • central nervous system injuries
    • encephalitides and meningitides (and their forms)
    • infections (of any form)
    • brain tumors

3. How to receive the medications?

  1. call the Humanitarian Center's hotline (0800 509 001) and leave an application
  2. answer the call of the Humanitarian Center's employee and find out what documents should be submitted and what pharmacy can the medicine be received at
  3. get medications at the pharmacy by presenting the necessary package of documents

4. Where medicines are issued?

Medicines can be obtained in pharmacies of Artemovsk, Kramatorsk and Mariupol.

5. What amount of medications is provided to a child?

Each child is provided with pharmaceuticals for three months ahead.

HEALTHY HEART

1. What kind of assistance is provided?

Endovascular treatment of heart defects using occluder.

2. What diseases is the assistance provided for?

  • patent ductus arteriosus;
  • secondary atrial septal defect.

3. Who can get help?

Children from 0 to 18 years old, living in Donetsk, Luhansk, Kharkiv and Zaporizhia regions and belonging to the following vulnerable categories: 

  • families raising orphans and children under custody; family-type orphanages;
  • large families with 3 and more children under 18;
  • single parents or widows/widowers with children under 18;
  • families in difficult life circumstances (families, who have more than one severely ill family member, for example, mother with cancer and her child with ICP, or two disabled persons of the 1st group etc);
  • families with total monthly income below UAH 10,000 (confirmed by a certificate from the tax office).

4. How to file an application?

The application can be left via the Humanitarian Center's hotline: 0800 509 001.

HEARING AID FOR CHILDREN

1. Who can be provided with it?

Families belonging to the following categories can request assistance:

  • Families with many children or one-parent families; family-type orphanages (FTO), orphans and children under custody; families raising more than one severely ill child; families with two or more disabled persons.
  • Children with newly diagnosed hearing loss. The age is up to five years old.
  • Internally displaced families or families living in non-government-controlled areas of Donbas.

2. How to file an application?

The application for a child's hearing aid can be left via the Humanitarian Center's hotline: 0800 509 001 or sent via email: helpdonbass@fdu.org.ua.

REHABILITATION OF INJURED CHILDREN

1. Who can be provided with this assistance?

Children under 18 from Donetsk and Luhansk regions.

2. What are the areas of assistance?

  • treatment of neurological diseases;
  • recovery of musculoskeletal system;
  • rehabilitative treatment of internal organs;
  • correction of state after operative long-term treatment;
  • arthroplasty;
  • limb and eye prostheses;
  • adaptive rehabilitation.

3. How long does the course last and where does it take place?

Rehabilitation course for a child lasts 21 days and takes place in one of rehabilitation centres of Zaporizhia, Berdiansk or Sloviansk.

4. How to file an application?

To sign up for a rehabilitation course it is necessary to make a call to the Humanitarian Center's hotline: 0800 509 001.

PEACEFUL SUMMER FOR DONBAS CHILDREN

1. Who can take part in it?

Children of 8-14 years old living in non-government-controlled areas and "grey zone" of Donetsk and Luhansk regions.

It is planned to improve health condition of 500 children. The priority will be given to children from multi-child or one-parent families.

2. Where and when can children go for vacation?

  • Health-improving Soniachnyi summer camp for children located in Sviatohirsk, Donetsk region. The shift lasts through July 9-22, 2017.
  • Health-improving Dolphin summer camp for children located in Skadovsk, Kherson region. The shift lasts through July 18-31, 2017.
  • Health-improving Albatros summer camp for children located in Strilkove settlement, Kherson region. The shift lasts through August 8-21, 2017.

3. How to file an application?

The application can be submitted:

  • On the website of Rinat Akhmetov Humanitarian Center (online help).
  • Via the Center's hotline: 0-800-509-001.

4. What documents shall be provided?

  • a copy of child's birth certificate;
  • a copy of one of the parent's passport with indicated registration;
  • a copy of one of the parent’s identification code;
  • a copy of IDP certificate (for IDPs);
  • certificate of epidemiological environment;
  • health certificate from medical institution (079/У form);
  • certificate/abstract of vaccination record (incl. the date of last vaccination), as follows:
    • BCG (vaccination against tuberculosis, at least 1);
    • DPR (tetanus, diphtheria and pertussis vaccination);
    • Polio vaccination (inactivated polio vaccine, oral polio vaccine);
    • MPR (Priorix) ‒ vaccination against measles, parotitis and rubella;
  • parents' consent for psychologists to work with their child (after approval in the project); 
  • parents' consent for taking photos and videos of their child as part of the project (after approval in the project);
  • a copy of large family certificate/document proving single mother status.
